Direct deposit is now an available option for the payment of workers’ compensation and death benefits. To comply with the proposed amendment of 12 NYCRR 300.26, Fire Districts of New York Mutual Insurance Company is providing a Direct Deposit Authorization Form DD-1.
To begin, change, or cancel the transmittal of workers' compensation benefit checks directly to a financial institution, please complete the DD-1 form and return the form to Lori Vega.
